Our office uses OptiMantra as our Electronic Medical Record system.  If you are a new patient with us please use the link below and choose "New Sign Up".  You can download the app or use your web browser (we find the web browser to be more user friendly!) 


OptiMantra messaging can be used to send messages directly to your provider, view your lab results, manage appointments, view superbills, and more.

Discounted Supplements

Our practice uses FullScript for medical grade supplements.  Once you are a patient your provider will send you an email with your invitation to join. 

Fullscript sells only medical grade supplements that are tested and vetted for quality sourcing, purity, and potency. This ensures that you are getting the best supplements available.


Your provider can give you direct recommendations within this platform and there are options for auto-refills.

SIBO Test Instructions

SIBO, or small intestinal bacterial overgrowth, is defined as the presence of excessive numbers of bacteria in the small bowel, causing gastrointestinal symptoms. The normal small bowel has lower levels of microbial colonization than does the colon. This normal balance is significantly disrupted with SIBO.  SIBO can be a complex result of various medications and conditions, such as IBS, Celiac, Crohn’s disease, bowel surgeries, and several other gastrointestinal complications. The symptoms of SIBO can arise from the malabsorption of nutrients, alteration in intestinal permeability, inflammation, and/or immune activation that arises from the abnormal bacterial fermentation within the small bowel.

GI-MAP Instructions

A GI-MAP stool test is a comprehensive diagnostic tool that analyzes the DNA in a stool sample to evaluate the health of the gastrointestinal tract. It uses quantitative polymerase chain reaction (qPCR) to identify and quantify bacteria, fungi, parasites, and viruses, and also assesses markers of intestinal health, digestion, and immune function. The results can help identify imbalances in the gut microbiome and inform personalized treatment plans for various digestive and other health issues.
